| Recently the demand for blood donations has increased. In order to reach this demand, Stanford Blood Center has started the Give blood for life! program for our blood donors. It provides a place for you to request appointments, access personal donation history, and choose from an assortment of new thank you gifts. It also enables us to let you know about local patient needs when your blood type is in demand, and to let you know about special promotions and events. | |

| The program officially started on March 25, 2005. | |
| 3. | Why was I enrolled in the Give blood for life! program? |
| We based our initial enrollment criteria on your past donation history and will periodically be adding new members to the club. | |
| 4. | What are the benefits associated with the Give blood for life! program? |
| The purpose of Give blood for life! is to make your donation experience easier and more fulfilling. Your membership will allow you to have online access to your donation history on our Web site and it will give you the capability of requesting a donation appointment online, which is then confirmed after you are contacted by one of our representatives. The program aims to keep you informed of special events and gives you an opportunity to earn points for customized Stanford Blood Center donor appreciation items. | |

The program has three levels: at least twice in the past year. three times in the past year. four or more times in the past year. At the end of each program year, you will receive notification of your level achieved. Additionally, your current level will be shown on your online donor history statement. |

| No, you will still be a member of Give blood for life! As always, we will be happy to assist you in scheduling your next donation or answering your questions. The benefits of your membership will still be honored. | |
| 7. | I’m a Four Seasons or Crimson Donor. Will I still be a club member if I join Give blood for life!? |
| Yes. We will continue to offer these clubs as a way to encourage your regular donation schedule. We may even contact you through the Give blood for life! program to inform you of special news about the Four Seasons club or Crimson Donor program. | |
| 8. | How does the point system work? |
| You will be awarded 100 points for each donation visit. When you log on to the Give blood for life! section of the Stanford Blood Center Web site, you will be able to see how many points you have and shop in our online store to redeem the points. | |
| 9. | What about the gallon gifts you have always given? |
| We hope that most of our donors will enjoy the Give blood for life! program and will choose their thank you gifts in our online store. However, we will still be offering our usual gallon gifts at our blood donation sites during this transition phase. | |
| 10. | What happens if I do not give as many times as I did last year; am I no longer part of Give blood for life!? |
| You are encouraged to donate as many times as possible for the good of our community. Once you are a part of Give blood for life! you will not be removed unless you request to be. You move from one level to another based on your donations for that program year. You accumulate more points towards the online store with more donations. | |
| 11. | How long is a program year? |
| Your program year starts from the first day of your enrollment into the club. For example, if you start the program on January 1, 2005, your membership will automatically renew on January 1, 2006 and will continue to automatically renew. | |

| Just let us know and we can remove you from the program at any time. | |
| 13. | How do I log on to the Give Give blood for life! Web site? |
| To access the Give blood for life! Web site go to http://bloodcenter.stanford.edu and click on the Give blood for life! tab in the menu bar. At the bottom of the page click the "Members Login Here" box. You have two options to log in. You can either enter the username and password provided on your membership letter or you can enter your Stanford Blood Center donor identification number and birth date. On your first login you will be prompted to change your username and password so that you have the ability to make them more memorable to you. | |

| New members that enroll online will be added to the Give blood for life! program every month. If you have registered online, you will be receiving a confirmation letter that explains the Give blood for life! program and outlines the benefits associated with being a member. | |
| 15. | I heard about Give blood for life! and I am interested in being a member. How do I participate? |
| You can enroll yourself into Give blood for life! If you have access to the Internet you can go to our Web site at http://bloodcenter.stanford.edu and click on the Give blood for life! tab in our menu bar. Then simply click on the "Enrollment Form" option. Fill in the form and follow the on-screen instructions. If you do not have access to the Internet, you have two other options. You can call our Customer Service number at 1-888-723-7831 for assistance or request a paper version of the "Enrollment Form" from our registration staff at the time of your next donation. | |

| If you are having trouble logging in with your username and password, please try to log in using your Stanford Blood Center donor identification number, which will be on your Donor ID/blood type card, and birth date. If you do not know your donor identification number, please call our Customer Service number at 1-888-723-7831 and our staff can help you. | |
| 17. | I would like to make an appointment for a mobile blood drive. How can I do that? |
| When requesting a donation appointment you will be given the choice of donating at either a donor center or a mobile blood drive. When you open the mobile drive drop box, you can choose any one of the mobile drives and locations that will best suit your schedule. | |
| 18. | I have forgotten my username and password. Where can I get that information? |
| At the login screen, click on the link that says "click here if you have forgotten your username and password." Enter your Stanford Blood Center donor identification number and birth date from your Donor ID/blood type card and click the "Get user login/password" link. | |
| 19. | How can I change my username, password, or contact information online? |
| After logging into the Give blood for life! Web site you can change your password by clicking the "Change Password" link in the left menu bar. A new window will open and you can enter your new password and then click "Change Password." To change your username or contact information, click the "Personal Information" link in the left menu bar. Change any of the information you need to and then click "Update Information." | |
| 20. | Can I choose how I am contacted? |
| Yes, when all "Give blood for life!" members first sign on to the Web site, they will be given their choice of preferred contact method; email, phone, or direct mail. | |
| 21. | How can I keep up to date on special events and promotions at the blood center? |
| We encourage all "Give blood for life!" donors to stay active and to participate in any of the many special events at the blood center. As an effort to help keep you connected, SBC will contact you with news and upcoming events that we will be holding. In addition, news and events will be regularly updated on the SBC webpage at http://bloodcenter.Stanford.edu. | |
| 22. | Who is eligible for the Give blood for life! program? |
| All SBC donors are eligible to be a part of the Give blood for life! program. If you do not donate with us within 18 months of your eligibility date, you will be considered an inactive donor and will lose your store points. You will be reminded when your points are about to become inactive. We will continue to contact you as a Give blood for life! member unless you request to be removed from the program. | |
